ROLE OF PHARMACEUTICAL CARE INTERVENTIONS IN IMPROVING MEDICATION ADHERENCE AND CLINICAL OUTCOMES AMONG CORONARY HEART DISEASE PATIENTS: A REVIEW

Abstract

Coronary heart disease (CHD) remains one of the leading causes of morbidity and mortality worldwide and represents a major public health challenge. Increasing prevalence of hypertension, diabetes mellitus, dyslipidemia, obesity, smoking, and sedentary lifestyle has contributed significantly toward the growing burden of CHD, particularly in developing countries. Long-term management of coronary heart disease requires continuous pharmacotherapy, lifestyle modification, regular monitoring, and adherence to prescribed treatment. However, medication non-adherence, polypharmacy, adverse drug reactions, and inadequate patient awareness continue to compromise therapeutic outcomes among CHD patients. Pharmaceutical care interventions provided by clinical pharmacists have emerged as an effective strategy for improving medication adherence, optimizing pharmacotherapy, preventing drug-related problems, and enhancing quality of life among cardiovascular patients. The present review summarizes the role of pharmaceutical care interventions in coronary heart disease management, factors affecting medication adherence, impact of pharmacist-led interventions on clinical outcomes, and the importance of multidisciplinary healthcare approaches in improving patient-centered therapeutic outcomes. The review further highlights the growing importance of clinical pharmacists in cardiovascular healthcare services and emphasizes the need for integration of pharmaceutical care into routine CHD management.
